Immunization Records Requests
¾Ã¾ÃÈÈ retains student immunization compliance records indefinitely. Current and former students may request a copy of their immunization records by completing the Immunization Records Request Form.  Requests are usually processed within 30 days.
Student medical information, including immunization records, will not be released without written permission except in life-threatening emergencies or as required by federal, state and local law. In response to requests from anyone other than the student, including parents and family members, the Student Health Insurance and Immunization Compliance Office cannot release medical information without written authorization from students age 18 or older. A signed authorization form must be submitted each time a student chooses to share information.
The university requires that all requests for Immunization Records be submitted by completing the

- Requests are usually processed within 30 days. The is only for current and former students seeking immunization records submitted during their initial matriculation process to the university.
- The Immunization Records Request Form is not available for incoming (first-time matriculating) students. ¾Ã¾ÃÈÈ matriculating for the first time must submit vaccine/immunization documentation to the . No other form of submission can be accepted.
