Faculty/Staff are required to submit IRS form 1040 annually to the Office of Human Resources. When tuition remission is requested for a legal spouse, qualifying domestic partner and dependent children, a copy of the IRS form 1040, listing the name of the student, will be accepted as certification. Please submit the IRS form 1040 to the Office of Human Resources before completing the tuition remission request in OnTrack.

These award are given to non-消消犯 staff in each of the following categories: Physical Plant, Office Staff, and Administrative Staff. Once the nominations are received, Human Resources will categorize the nominations into the appropriate job group. 

Nominees must be classified as full-time (work at least 30 hours per week) and have been in their positions at least one full year. Any university faculty member, staff member, or student may make a nomination. Nominations can be made in any or all of the three categories represented.

You will assist the Selection Committee greatly by giving accurate and professional responses about your nominee’s qualifications for this award. Please be concise in your comments, including any experiences with the nominee that you feel are relevant. Keep in mind that you want to convince the selection committee that your nominee deserves to be recognized as one of the university’s Staff Member of the Year recipients. Nominations are kept confidential throughout the selection process.

The Phoenix Rising Award is presented to one newer member of the Elon staff (less than five years of service) who has consistently displayed excellence in pursuit of the Elon Commitment with an engaged mind, inspired leadership and global citizenship;

 

The Phoenix Community Engagement Award is presented to a member of the Elon staff who has demonstrated exemplary service to the university, local or global community through their engagement in volunteer organizations.

The Phoenix Innovation Award is presented to a member of the Elon staff who has displayed innovation by creating, designing, improving or implementing a new idea or program that benefits the university or our community.

The deadline for submitting nominees is March 29, 2019. Paper copies are available in the Office of Human Resources if needed.

Get the Facts

Mammography is a specific type of imaging that uses a low-dose X-ray system for examination of the breasts. It allows a radiologist to identify differences between a normal breast and one that may show signs of cancer.

When to Get One
Women in their 20s and 30s should have a clinical breast examination as part of a periodic health exam by a medical professional every three years. After age 40, women should have a breast exam by a health professional every year. The U.S. Preventative Services Task Force recommends that women ages 50 to 74 should get a mammogram every two years.

The risk of getting breast cancer increases with age, so as long as a woman is in good health, she should continue getting routine mammograms. Young women at high risk for breast cancer should discuss with their doctors if earlier screenings are necessary.

Questions to Ask
The following are suggestions for ensuring you will receive a quality mammogram:
• Ask to see the FDA certificate issued to all facilities that meet high professional standards of safety and quality.
• Use a facility that either specializes in mammography or performs mammograms often.
• Bring a list of the places, dates of mammograms, biopsies, or other breast treatments you may have had before.

BCBSNC changes mail order prescription program /u/news/2017/10/05/bcbsnc-changes-mail-order-prescription-program/ Thu, 05 Oct 2017 11:50:00 +0000 /u/news/2017/10/05/bcbsnc-changes-mail-order-prescription-program/ BlueCross Blue Shield of North Carolina has mailed two notifications notifiying a change effective Sept. 30 that PrimeMail recently combined with Walgreens Mail Service. PrimeMail prescritions will be filled by Walgreens Mail Service starting Sept. 30, 2017.

Almost everything will else stays the same, including the following:

  • Your mail service phone number will remain the same
  • The Customer Service hours of operation are still still 24 hours a day, seven days a week
  • You will continue to receive refill reminders

What you need to know:

  • You’ll see the new name on your pill bottles, invoice and pharmacy benefit communications.
  • When it’s time to refill your prescription(s), it will be filled by Walgreens Mail Service.
  • To see prescription information or request a refill (if your prescription allows them), visit Walgreens.com/PrimeMail or call the number on your prescription label.
  • To protect your financial data, Prime did not move your credit card information. You will need to provide a credit card when filling your first prescription.
  • Validate your email address is on file.
  • Validate your over-the-counter medications, allergies and health conditions when you go online or speak to a call center agent when ordering your prescription(s).
  • If you don’t have refills, or if your prescription expired, ask your doctor to send a new prescription electronically to Walgreens Mail Service in Tempe, AZ or fax to 1-800-332-9581.
  • The Auto Refill program is now available! Sign up online or tell the call center agent which medications you want in this program.
